Update README structure: Extract and Scrub PII, Upload

Browse Source 
Changed from 'Customer Workflow' and 'LangChain Team Workflow' to:
- Extract and Scrub PII (customer actions)
- Upload (LangChain team action)

Clearer separation of responsibilities.
