docs: mark cart-issue-triage table as historical — all six issues closed

The table still read 'STILL REPRODUCES (worst offender)' for #138, which is
closed and no longer reproduces: 7200 frames clean with no watchdog
signature, and 5200 clean under scripted run+jump input. That stale verdict
sends readers to chase a fixed bug. Banner the table as a historical record
and record the re-verification.
