docs(app): phase 5 evidence, the G3 halt and phase 3 gate rows (#1039) (#1133)

- Halt, mode A vs mode B: 0 underruns in each.
- Phase 3 Studio gate: two quiet-host passes (halted p99 about 18 ms). One run under load failed, and one was skipped.
