fix(hearth): cap attachments at 10 MB, scrub filenames

- 3-layer size check (metadata, response headers, body) prevents OOM from oversized uploads
- chunked base64 encode replaces fromCharCode(...buf) stack blowup
- sanitizeFilename() strips control chars, caps at 120 — blocks terminal-escape + prompt injection
