fix(theme): eliminate dark-on-dark headings, theme postboxes, widefat tables, and form controls
- Broaden CSS custom properties to :root and body shells - Enforce pure white color (#ffffff) on all h1-h6 headings across all EMCP screens - Theme WordPress postboxes to dark metal cards (#111116) with dark headers (#16161d) and white titles - Theme widefat and wp-list-table with dark zebra striping and high-contrast links - Style all form controls (input, select, textarea) and buttons to Blood & Steel palette - Elevate description text to readable #a1a1aa and labels to #ffffff - Remove nested .wrap classes in page-migrate.php and page-memory.php - Bump EMCP_TOOLS_VERSION to 3.14.5
